Must include the following details:

Applicant's name (Surname, First Name, Middle Name)

Contact Number

Written in BOLD LETTERS on the upper left corner

Two (2) Copies-Photocopy of passport bio-page

Two (2) Copies-4.5CM (HIGH) X 3.5CM (WIDE) white background

Photo must be taken within the last three (3) months no black and white photos the head should take 70-80% of the photo.

• Mayor’s Permit or Business Permit.

• Income Tax Return (ITR).

FOR STUDENTS

• Proof of enrollment

• School ID

FOR RETIREES

• Pension Statement of the latest six (6) months

• Proof of regular income generated by a property for the last six (6) months
