Photocopy of Passport Bio-page (page 2)

Original & Photocopy of valid visa/s and arrival stamps to OECD member countries for

the past 5years (If applicable only)

Original Certificate of Employment

(must include applicant's position, date hired, compensation, office address, HR

landline number(cell-phone number is not allowed), HR e-mail address)

Original Personal Bank Certificate

(must include account type, current balance, account opening date, ADB)

Bank Statement

(original or certified true copy of bank statements/passbook for the last 3 months)

Photocopy of ITR (Income Tax Return) or Form 2316 Copy of PRC Card or IBP Card (If applicable only)

PSA Birth Certificate and/or Marriage Certificate if travelling with family.

Processing Time

Approximately 10 working days from the time the application is received by the embassy (can be extended without prior notice).

FOR BUSINESSMAN

• Photocopy of Business Registration from SEC or DTI

• Photocopy of Business Permit or Mayor's Permit

• Original Personal Bank Certificate (must include account type, current balance, account opening date, ADB)

• Photocopy of ITR (Income Tax Return) or Form 2316

FOR STUDENTS

• School Certificate (Original)

• Copy of School ID

• Original PSA Birth Certificate

• Parents' Documents

FOR RETIREE

• Original Personal Bank Certificate (must include account type, current balance, account opening date, ADB)

• Original & Photocopy of valid visa/s and arrival stamps to OECD member countries for the past 5 years (If applicable only)

IMPORTANT REMINDERS

• Please print the application form in A4 size.

• No staple allowed to all requirements.

• Please submit complete documents to avoid delays in processing.

• Please do not leave any blank in the application form. Kindly put “N.A.” if it’s not applicable.

• Make sure that the back page of the passport is filled up (Address, Tel Numbers, and Name of Person to be contacted in case of emergency).
